At Veterans Park turn left between the old court house and the park. The new court house is behind the old court house. First visit the probate office, along with valid id. They charge $15 dollars for the "application". Seems like they would accept cash or check. I took cash to be safe. Then, hand carry form to county jail. I believe the sheriffs office only accepts cash or money order. Once again, carry cash in exact change to be sure. They charge $29. The jail has the electronic finger printing machine. They only finger print between 9-11, and 1-4 M-F. Be forewarned that if they are processing a prisoner, you'll have to wait.
